Job Summary:
Position is responsible for coordinating the patient centered medical home, collaborating with clinicians, nursing and support staff to help ensure the smooth progression and throughput of daily patient schedules. Act as a liaison between clinics, hospitals and community resources helping patients receive prompt access and continuity of care. Provide clinical assessment of walk-in patients. Provide support and assistance with direct patient care and be a resource for Medical Assistants and nursing staff demonstrating the ability to lead change and act independently.
Must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations.
Required Experience & Skills:
Qualified candidate will be a licensed LPN in Colorado. Must have intermediate knowledge and understanding of information technology associated with Electronic Health Records.
Bilingual skills preferred
Required Education:
High School Diploma or Equivalent
Associate Degree in Nursing (ADN)
Certifications:
Colorado LPN License
BLS certification
Requirements and Conditions:
