This letter is to inform you that our medical provider, Amy Leibli, DNP, will be leaving our organization on October 13, 2020. Amy has been a valued member of our provider staff for years, and we wish her great success in her next venture.
We know that you may be concerned about your future healthcare needs with Amy’s departure. Valley-Wide Health Systems has many skilled providers available for your healthcare needs. Since you are a current patient within the Valley-Wide organization, transition to a new provider will be simple. All Valley-Wide locations have access to your records through our Electronic Health Records system, so you can seamlessly transition to appointments with any of our other providers. We can also see you at any other Valley-Wide clinic should you choose a provider at a different clinic location.
If you would like to browse through the listings of all our providers, you can visit our website at www.valley-widehealth.org/providers. We have a new provider, Cooper Couch, PA-C, starting in November. He will be available in addition to the providers listed on our website.
We feel privileged to work with many wonderful patients and we appreciate that you have chosen us to help you with your healthcare needs. If you choose to seek care elsewhere, you may request that copies of your records be forwarded to your new primary care provider. Please contact us with any questions or concerns.
