Official notice is given that sealed Responses to Request For Qualifications from architectural firms will be received in the Administrative Services Building, Valley-Wide Health Systems, Inc. (“VWHS”), 128 Market Street, Alamosa, CO 81101, until 2:00 P.M. local time on January 28th, 2016, for the following:
REQUEST FOR QUALIFICATIONS: ARCHITECTURAL SERVICES FOR REMODEL DESIGN OF 531 MAIN ST., CANON CITY, CO 81212. Responses shall consist of a Statement of Interest / Response to RFQ that contains the requested information and data listed in the Request For Qualifications document, a copy of which can be obtained from VWHS’ Construction Manager (call: (719) 587-1126 or email firstname.lastname@example.org) A copy, complete with pictures of the building, can also be found (below) at valley-widehealth.org. VWHS seeks to evaluate architectural firms that have proven experience in commercial remodel and ambulatory medical and dental clinic design.
The Project consists of the design and remodel of the 531 Main Street, Canon City, CO (1902 Baker and Biggs Building), which will be a combination medical and dental clinic serving the residents of Fremont County, CO. Submission of all Statements of Interest / Responses to RFQ shall be in sealed envelopes and shall be received by VWHS prior to the aforementioned date and time or such submittals shall be subject to disqualification by VWHS. The submittal deadline may be extended by VWHS, in its sole discretion. If extended, all responding parties will be notified in writing and will have an opportunity to re-submit their Statements of Interest/ Responses to RFQ, with any additional or updated materials, within the extended time period. VWHS reserves the right to reject any and all Statements of Interest / Responses to RFQ or parts thereof, to waive any irregularities or informalities in these procedures, and to award the professional services agreement in a manner best serving the interest of VWHS.
