On behalf of Vanessa Zwegers, FNP, Valley-Wide is providing this letter to inform her patients that she will be leaving Sierra Blanca Medical Center as of November 8, 2021. Vanessa will be one of Valley-Wide Health Systems, Inc. full time providers at our Convenient Care Community Clinic providing access for acute care services.
Our care teams will be available to continue to provide care and services to you and your family. As a current patient within the Valley-Wide organization, transition to a new provider will be simple as all Valley-Wide providers have access to your records through our Electronic Health Records system. We encourage you to call to schedule future appointments through any of Valley-Wide’s healthcare clinics at 1(833)-350-1113. Please reference our website at www.valley-widehealth.org/providers for a current list of providers.
There are providers available to your to continue using Valley-Wide Health Systems for your healthcare needs. Please inform our office staff which provider you will be transitioning your care to so your “Primary Care Provider” can be changed in your electronic health record when scheduling your next appointment.

If you choose to transition your care to a provider outside of Valley-Wide, you may request a release of medical records and provide the name and address of the provider or practice that you wish to continue your care with. This process is also simple as your records are available through an Electronic Health Record system.
