In an effort to improve access and effectiveness of healthcare services in the San Luis Valley, Valley Wide Health Systems, Inc. and the San Luis Valley Behavioral Health Group (formally SLV Mental Health) are partnering to provide residents of the San Luis Valley access to needed services. The goal of this partnership is to provide the right service at the right time at the right place.
The core of this approach is behavioral health consultants working with primary care partners towards a whole person orientation where the health care system supports team-based care and provides case management and support to individuals to meet their self-management goals.
You may not be aware of the services the Behavioral Health Group provides, but we encourage you to explore any needs that you may have in this area in addition to caring for your physical health. We know that physical health care is impacted by your emotional health and well-being and we want to see our community be the healthiest it can be!
