Valley-Wide attempts to engage patients by offering opportunities to provide feedback through various methods, including surveys and Patient Family Advisory Councils (PFAC). PFACs are a mechanism to gather input around the general experience of patients and their family members during an organized meeting.
Advertisements for these opportunities with dates and meeting times can be found in our clinics. If you do not see a PFAC opportunity advertised in your clinic, please request information from the front desk staff or your provider.
If you are unable to attend these meetings, clinic staff will also solicit individual feedback from patients and family members at the time of their visit. We understand that formal meeting times may not be convenient with family schedules and work requirements.
We look forward to hearing from you!
