Valley-Wide Health Systems, Inc. (Valley-Wide) was selected by The Colorado Health Foundation to participate in the Team-Based Care funding opportunity and was awarded $150,000 in grants. The model is designed to improve the value of health care teams. The Colorado Health Foundation and Valley-Wide are focused on improving and maintaining Coloradans’ primary care and overall health by investing in practice transformation.
The funding opportunity began September 1st, 2015 and will end August 31st, 2018. During this time, Valley-Wide will be focusing on the roles that each health care team member plays in patient care. Team members will help encourage patients to make informed decisions about their care and health goals. Valley-Wide believes that a relationship between a patient, the primary care provider (PCP), and the care team creates a trust that leads the patient in better health care decision-making and better health outcomes.
