Valley-Wide Health Systems, Inc. (Valley-Wide) is proud to report that it is a recipient of two federal Health Resources and Services Administration (HRSA) Fiscal Year 2015 Quality Improvement awards, which are intended to recognize Community Health Center quality achievements.
- National Quality Leader: $41,921 – For health centers that are the highest performers compared to national standards and benchmarks in key clinical areas. Valley-Wide exceeded national clinical benchmarks (Healthy People 2020 objectives and health center national averages) in areas such as chronic disease management, preventive care, and perinatal/prenatal care, demonstrating the critical role that health centers play in promoting higher quality health care nationwide. Valley-Wide was one of only two health centers in Colorado to receive funding for the National Quality Leader category.
- Clinical Quality Improvement: $21,921 – For health centers that have demonstrated improvements in one or more clinical measures. Valley-Wide demonstrated at least a 10 percent improvement in clinical quality measures between 2012 and 2013, showing a significant improvement in the health of the patients it serves.
Valley-Wide is a Community/Migrant Health Center system with twelve medical clinics located throughout southern Colorado, in the San Luis Valley, the Lower Arkansas Valley and in Canon City. Valley-Wide also has seven dental clinics located in the San Luis Valley and the Lower Arkansas Valley, and physical therapy clinics located in Alamosa, Monte Vista, Antonito and San Luis. Many medical clinics have behavioral health practitioners co-located for ease of integrated care for patients.
